To provide you with the most effective legal advice during your consultation, we ask that you complete our Client Intake Form before your scheduled appointment. This form helps us gather important background information so that our time together can focus on solutions, not paperwork.

If you have documents related to your matter (contracts, leases, correspondence, etc.), please upload them with your form or bring copies to your consultation.

Submit the completed form at least 24 hours before your appointment to allow us time for review.
Confidentiality Reminder
All information you provide in the intake form is kept strictly confidential. Completing the form does not create an attorney-client relationship until an engagement agreement is signed, but your details will never be shared outside our firm.
